This page will tell you about the work of the Long Term Conditions Clinical Network. This is one of three clinical networks formed by Hutt Inc to make sure people in our community have primary and secondary health services which work well together to give them the care they need.
At present we are working to:
- implement the 3DHB self-management support framework for people with long term conditions and strengthen the self management support programmes in our community
- implement the Long Term Conditions programme in primary care
- establish a Diabetes Clinical Network to identify service improvements and provide oversight to Diabetes outcomes in our community
- transform the respiratory model of care across the system. We have called this the Respiratory Patient Journey Project.
