This page will tell you about the work of the Acute Demand Clinical Network. This is one of three clinical networks formed by Hutt Inc to make sure people in our community have primary and secondary health services which work well together to give them the care they need.
At present we are working to:
- develop and implement a system wide winter plan. This includes multiple projects that work to keep people well over and reduce acute demand over the winter period.
- implement Primary Options Acute Care (POAC) that allow more services to be delivered in a primary care setting.
- continue and refine the role of the PACC coordinator in the Hutt Valley
