Hutt Valley DHB Mental Health and Addiction Services Proposed Single Point of Entry
The single point of entry function will be finalised when our planned working party involving consumers, cultural advisors, clinicians, primary care, non-government organisations and administration staff complete their work later this year. This work will build on the service’s new alignment to our local primary health care organisations.
What are we proposing for the single point of entry?
The single point of entry model will support our new holistic patient and family centered model of care across the mental health service.
A multi disciplinary patient based assessment and/or triage process will clearly identify and prioritise the client’s needs and matching those needs to the right care.
To ensure a 24 hour a day, seven day a week access, the single point of entry function will be managed as part of the intensive service.
